A patient presents with a 2.5cm simple repair of the right arm and a 3.0cm simple repair of the right hand. Per 2026 CPT guidelines, how should these be reported?
Show answer →Lengths of repairs are summed when they are the same classification (Simple) AND fall within the same anatomical grouping. For Simple repairs, the Trunk and Extremities group includes the hands and feet. Total length: 5.5cm. Code: 12002 (Simple repair of superficial wounds of scalp, neck, axillae, external genitalia, trunk and/or extremities [including hands and feet]; 2.6 cm to 7.5 cm). AHIMA Tip: Always verify if the specific anatomical site is included in the grouping for that classification level.
A patient undergoes a 5.0cm intermediate repair of the scalp and a 3.0cm intermediate repair of the neck. How are these codes sequenced and reported?
Show answer →Intermediate repairs for the scalp and neck fall into DIFFERENT anatomical groups. Scalp is in the Scalp, axillae, trunk and/or extremities (excluding hands/feet) group (12031-12037). Neck is in the Hands, feet, neck and/or external genitalia group (12041-12047). Report 12032 (Scalp 5cm) and 12042-51 (Neck 3cm). Sequencing: The code with the highest RVU (typically the longer/more complex) is listed first. AHIMA Distractor: Summing these would be incorrect because the anatomical groups differ.
The physician uses only adhesive strips (e.g., Steri-Strips) to close a 2.0cm superficial laceration on the forehead. How is this service reported in CPT?
Show answer →This is reported with the appropriate Evaluation and Management (E/M) code, not a wound repair code. CPT guidelines state that wound closure using only adhesive strips as the sole repair material does not qualify for the 12001-13160 repair codes. If tissue adhesive (e.g., Dermabond) or sutures/staples were used, a repair code would be appropriate. Note: In HCPCS Level II, G0168 may be used for Medicare, but for the CCS (CPT focus), E/M is the standard answer for strips-only.
A patient has a 10cm simple repair of the leg and a 5cm simple repair of the trunk. How are these reported?
Show answer →Sum the lengths for the same classification and anatomical group. Both the leg and trunk are in the Scalp, neck, axillae, external genitalia, trunk and/or extremities (including hands and feet) group. Total: 15.0cm. Code: 12005 (Simple repair; 12.6 cm to 20.0 cm). AHIMA Distractor: The exam may offer 12004 (7.6-12.5cm) and 12002 (2.6-7.5cm) separately with a -51 modifier. Do not choose this; CPT instructions explicitly require summing same-group/same-class repairs.
A 5cm intermediate repair of the face and a 4cm simple repair of the nose are performed. How are these reported and sequenced?
Show answer →Intermediate Face (12052) and Simple Nose (12013). Note: While the face and nose are in the same anatomical group for both classes, they are DIFFERENT classifications (Intermediate vs Simple). Therefore, they are NOT summed. Sequence the higher-level repair first: 12052, then 12013-51. CCS Strategy: On the exam, verify the anatomical grouping for BOTH classifications; face/nose are grouped together in both simple and intermediate, but the classification difference prevents summing.
