Sample CSFA Practice Questions

Question 1

During a midline laparotomy, the CSFA assists in identifying layers. After the skin and subcutaneous tissue (Camper's and Scarpa's fascia), what is the next anatomical structure encountered before reaching the peritoneum?

Show answer →
Answer

The linea alba. This is a fibrous structure formed by the fusion of the aponeuroses of the external oblique, internal oblique, and transversus abdominis muscles. In a midline incision, it serves as the primary landmark to avoid muscle fibers. Clinical Pearl: Below the umbilicus, Scarpa's fascia is more distinct and often requires separate closure in obese patients to prevent seroma. CSFA Tip: NBSTSA often tests the sequence of layers from superficial to deep.

Question 2

When making the transverse Kocher incision, which neck muscle is the first to be encountered and must be divided or retracted to reach the deep cervical fascia?

Show answer →
Answer

The Platysma muscle. It is located within the subcutaneous tissue. Surgical Note: The SFA assists in creating subplatysmal flaps superiorly to the thyroid notch and inferiorly to the suprasternal notch to provide adequate visualization of the strap muscles and the surgical field.

Question 3

A patient undergoes a radiofrequency ablation of the facet joints. What is the formal anatomical name for these joints, and what type of joint are they?

Show answer →
Answer

Zygapophysial joints (or Z-joints). They are synovial, plane-type joints between the superior and inferior articular processes of adjacent vertebrae. Rationale: They facilitate and guide movement while limiting excessive rotation and shear. They are innervated by the Medial Branch of the Posterior Primary Rami. CSFA Tip: Understand that hypertrophy of these joints (facet arthropathy) contributes significantly to lateral recess and foraminal stenosis.

Question 4

During exposure of a tibial fracture, the SFA is instructed to minimize periosteal stripping. What are the two distinct layers of the periosteum and their respective functions in bone repair?

Show answer →
Answer

The periosteum consists of: 1. Outer Fibrous Layer: Highly vascularized, contains nerves, and serves as an attachment point for tendons and ligaments. 2. Inner Osteogenic (Cambium) Layer: Contains progenitor cells (mesenchymal stem cells) that differentiate into osteoblasts. NBSTSA Tip: The cambium layer is responsible for appositional growth and the formation of the external callus during fracture healing. Excessive stripping removes the biological machinery needed for repair, significantly increasing the risk of non-union.

Question 5

Assisting in a posterior approach to the hip, the CSFA must help identify the key to the deep gluteal region, which originates on the anterior sacrum and inserts on the superior border of the greater trochanter. Name this muscle.

Show answer →
Answer

The muscle is the Piriformis. Action: Lateral (external) rotation of the extended thigh and abduction of the flexed thigh. Clinical Significance: The Sciatic Nerve typically exits the pelvis inferior to the piriformis muscle. CSFA Exam Tip: The piriformis is a critical landmark in posterior hip surgery. Piriformis Syndrome involves sciatic nerve compression. NBSTSA may ask about the Short External Rotators of the hip, which include the Piriformis, Gemelli (Superior/Inferior), Obturators (Internus/Externus), and Quadratus Femoris.
