Sample NCLEX-PN Practice Questions

Question 1

A client reports their last menstrual period (LMP) began on October 14, 2025. Using Naegele's rule, what is the estimated date of delivery (EDD)?

Show answer →
Answer

ANSWER: July 21, 2026. RATIOANLE: Naegele's rule calculation: Subtract 3 months from the first day of the LMP, add 7 days, and add 1 year (LMP - 3 months + 7 days + 1 year). October 14 minus 3 months = July 14; July 14 plus 7 days = July 21. CLINICAL PEARL: This rule assumes a standard 28-day cycle. On the NCLEX-PN, ensure you do not accidentally add or subtract the wrong number of days. If the LMP is in January, February, or March, you simply add 9 months and 7 days.

Question 2

A postpartum client's fundus is firm, 2 cm above the umbilicus, and displaced to the right. Which nursing action is the priority?

Show answer →
Answer

The client likely has a distended bladder. A full bladder displaces the uterus upward and to the side (usually the right), preventing effective uterine contraction and increasing the risk of postpartum hemorrhage (PPH). The priority action is to assist the client to void. If unable to void, straight catheterization may be required. NCLEX Tip: Always check bladder status when the fundus is not midline or is higher than expected. In BUBBLE-HE, B (Bladder) directly impacts U (Uterus).

Question 3

A newborn at 1 minute has a heart rate of 110, slow/irregular respirations, some flexion of extremities, a grimace when suctioned, and a pink body with blue hands and feet. Calculate the APGAR score and identify the level of distress.

Show answer →
Answer

Score: 6. Breakdown: HR >100 (2), Resp slow/irreg (1), Tone some flexion (1), Reflex grimace (1), Color acrocyanosis (1). A score of 4-6 indicates moderate distress. NCLEX Tip: Acrocyanosis (blue extremities) is common and only loses 1 point. Scores are calculated at 1 and 5 minutes to evaluate transition.

Question 4

A client with depression states, I won't have to worry about my bills much longer. What is the LPN's immediate priority action?

Show answer →
Answer

The LPN must immediately assess for suicidal ideation (SI) and a specific plan. Direct, non-judgmental questions like Are you thinking of killing yourself? are essential. Rationale: Vague statements often mask suicidal intent. Safety is the priority. NCLEX Tip: NCSBN emphasizes identifying lethality. High-lethality methods include firearms, hanging, or jumping. The LPN must report findings to the RN and provider immediately and ensure the client is never left alone (1:1 observation).

Question 5

A post-thyroidectomy patient reports a tight feeling in the neck. The PN notes increased serosanguinous drainage and frequent swallowing. Which cue is the highest priority to report immediately?

Show answer →
Answer

Frequent swallowing is a critical cue indicating potential concealed hemorrhage tracking down the esophagus. While a tight feeling and drainage are important, frequent swallowing often precedes airway obstruction from a hematoma. PN Action: Assess the back of the neck for pooling blood and notify the RN/Provider. Strategy: NCSBN emphasizes safety and ABCs; airway obstruction is the primary risk here.
