Preparing for the NREMT-P (National Registry Paramedic) exam? Diagnostics is one of the areas our deck drills hardest: 150 of its 1060 flashcards focus on diagnostics, covering topics like Alveolar Plateau, Sinus Bradycardia. During Phase III (Alveolar Plateau) of the capnograph, what physiological process is occurring, and where is the PETCO2 measurement specifically taken?
Show answer →Phase III represents the emptying of the alveoli, where CO2-rich air is exhaled. It is characterized by a nearly horizontal or slightly upsloping line. The PETCO2 value is measured at the very end of this phase (Point D), representing the peak CO2 concentration at the end of exhalation. Exam Strategy: NREMT often asks to identify the specific point on the waveform that represents the actual PETCO2 reading. It is the highest point of the waveform just before the rapid downstroke of inspiration (Phase IV).
A 72-year-old male presents with lightheadedness. HR 38, BP 88/50, diaphoretic. ECG shows Sinus Bradycardia. According to current ACLS guidelines, what is the first-line pharmacological intervention and the maximum total dose?
Show answer →Atropine 1mg IV/IO is the first-line medication for symptomatic bradycardia (AHA 2020/2025 updates). This may be repeated every 3-5 minutes to a maximum total dose of 3mg. If Atropine is ineffective, the paramedic should transition to Transcutaneous Pacing (TCP) or consider a chronotropic infusion (Epinephrine 2-10 mcg/min or Dopamine 5-20 mcg/kg/min). Exam Tip: NREMT often tests the escalation to pacing if pharmacological therapy fails or if the patient is severely unstable.
A 12-lead ECG shows a positive QRS in Lead I and a negative QRS in Lead aVF. Lead II is also negative. What is the axis deviation and a common clinical cause?
Show answer →Left Axis Deviation (LAD). The axis is between -30° and -90°. Common causes include Left Anterior Fascicular Block (LAFB), Left Ventricular Hypertrophy (LVH), inferior MI, or paced rhythms. NREMT Tip: If Lead I is positive and aVF is negative, check Lead II. If Lead II is also negative, it is pathological LAD; if Lead II is positive, it is physiological LAD (0 to -30°) which can be normal.
54yo male, history of DM, found unresponsive. Skin is diaphoretic, HR 110, BP 100/60. BGL is 38 mg/dL. Which part of AEIOU-TIPS is most likely, and what is the immediate priority?
Show answer →Insulin (Hypoglycemia). Priority: Dextrose (D10 or D50) or Glucagon. Hypoglycemia is the great mimicker of stroke. NREMT Strategy: Always check BGL in AMS before advanced neuro-imaging or psych labels. Current 2026 guidelines favor D10 (10% Dextrose) 100-250mL over D50 to reduce the risk of extravasation and post-treatment hyperglycemia.
45yo found unresponsive. RR 6, SpO2 82%, pupils 1mm. What is the most likely toxidrome and the classic triad of findings?
Show answer →Opiate Toxidrome. The Opiate Triad includes: 1. CNS Depression (unresponsive), 2. Respiratory Depression (low RR/tidal volume), and 3. Miosis (pinpoint pupils). Other findings include bradycardia, hypotension, and decreased bowel sounds. NREMT Tip: While miosis is classic, it may be absent in meperidine (Demerol) use or mixed ingestions. Respiratory depression is the most sensitive predictor of opioid toxicity.
