A 24-year-old trauma patient has a HR of 120, RR of 24, and BP of 110/70. Ten minutes later, the BP is 88/50. What clinical stage of shock has the patient entered?
Show answer →The patient has moved from compensated to decompensated shock. The hallmark of decompensated shock is hypotension (Systolic BP < 90 mmHg in adults). In compensated shock, the body maintains perfusion via the SNS (tachycardia, vasoconstriction). Once these mechanisms fail, BP drops. NREMT Tip: Do not wait for hypotension to treat for shock; it is a late and ominous sign. Treatment must begin during the compensated phase to improve outcomes.
During your primary assessment of a patient in shock, you note the patient is anxious and restless. What is the physiological cause of this mental status change?
Show answer →Anxiety and restlessness are early signs of compensated shock caused by cerebral hypoxia and the release of epinephrine/norepinephrine (SNS response). As shock progresses to the decompensated stage, mental status shifts from agitation to lethargy and eventually coma due to critical lack of brain perfusion. NREMT Tip: Impending doom is a classic exam descriptor for early shock.
You are treating a patient with suspected internal bleeding. The initial BP was 120/80. The repeat BP is 114/92. What does this change in pulse pressure indicate?
Show answer →A narrowing pulse pressure (the difference between systolic and diastolic) is an early sign of compensated shock. As cardiac output drops (lowering SBP), the body increases systemic vascular resistance via vasoconstriction, which raises the diastolic pressure (DBP). NREMT Tip: Narrowing pulse pressure often precedes a significant drop in systolic BP and is a high-yield indicator of early compensation.
Which clinical finding is considered the most sensitive and earliest indicator of hypoperfusion in an adult patient?
Show answer →Altered mental status (restlessness/anxiety) or tachycardia. While skin signs (cool/pale) are early, the brain and heart respond almost instantly to the catecholamine surge and slight drops in oxygenation. NREMT Tip: If the exam asks for the most sensitive indicator, look for mental status changes. If it asks for early vital sign changes, look for tachycardia.
A febrile patient presents with flushed, warm skin, a HR of 110, and a BP of 100/60. Why might this patient present with warm skin despite being in a state of shock?
Show answer →This is the warm or hyperdynamic phase of Septic Shock (Distributive). Bacterial toxins cause widespread vasodilation and increased capillary permeability. Because blood is being shunted to the skin initially, it feels warm/flushed. As it progresses to the cold phase, it mirrors hypovolemic shock skin signs. NREMT Tip: Sepsis is often missed because skin isn't always cool/clammy in early stages.
