Sample TCRN Practice Questions

Question 1

A TBI patient receives repeated Mannitol doses for ICP spikes. Which lab value is most critical to monitor to prevent acute kidney injury (AKI) and what is the threshold?

Show answer →
Answer

Serum Osmolality and the Osmolar Gap. Mannitol is an osmotic diuretic. If the Osmolar Gap (Measured Osm - Calculated Osm) exceeds 20 mOsm/kg, the risk of AKI increases significantly due to accumulation. BCEN tip: While Serum Osmolality >320 mOsm/kg is a traditional cutoff, the Osmolar Gap is a more specific indicator of Mannitol accumulation and renal risk. Monitor BUN/Creatinine closely.

Question 2

Contrast the traditional CRASH-2 dosing regimen for TXA with the updated 2024-2026 TCCC/TECC recommendations for massive hemorrhage.

Show answer →
Answer

Traditional (CRASH-2): 1g IV bolus over 10 minutes, followed by a 1g IV maintenance infusion over 8 hours. Updated/Tactical (TCCC): 2g IV/IO slow bolus (over 10 mins) as soon as possible, but not later than 3 hours. Rationale: The 2g bolus simplifies administration in high-stress environments, eliminates the need for a secondary infusion pump, and achieves therapeutic levels faster. BCEN may test both, so look for prehospital vs. in-hospital context. Rapid administration (<10 mins) must be avoided due to the risk of transient hypotension.

Question 3

An adult trauma patient requires tetanus prophylaxis. They have never received a Tdap vaccine, only Td boosters in the past. Which vaccine is preferred by the CDC/BCEN?

Show answer →
Answer

Tdap (Tetanus, Diphtheria, and Acellular Pertussis) is preferred over Td for any adult who has not previously received a dose of Tdap to provide protection against pertussis. Note: Tdap can be administered regardless of the interval since the last tetanus toxoid-containing vaccine.

Question 4

What is the mechanism of action of Sodium Thiosulfate, and why is it often administered alongside or after Hydroxocobalamin?

Show answer →
Answer

Sodium Thiosulfate acts as a sulfur donor for the endogenous enzyme rhodanese. Rhodanese converts cyanide into thiocyanate, which is non-toxic and renally excreted. While effective, its onset is slower than hydroxocobalamin. It is often used as an adjunct to enhance the total clearance of cyanide from the body. Note: It can be given through the same IV line as Hydroxocobalamin but should not be mixed in the same bag.

Question 5

In the initial resuscitation of a trauma patient with a suspected pelvic fracture and borderline blood pressure, why is Fentanyl often preferred over Morphine for rapid pain control?

Show answer →
Answer

Lack of histamine release and minimal hemodynamic impact. Rationale: Morphine causes histamine release, which can lead to peripheral vasodilation and hypotension. Fentanyl is more lipid-soluble (faster onset) and does not cause significant vasodilation, making it safer in the hemodynamically borderline trauma patient.
